Empath Health, a leading provider of Hospice services in Sarasota and Manatee counties, opened a new facility in Bradenton this month to provide essential healthcare, social services and support to seniors who are eligible for a nursing home but wish to remain in their homes.
Empath LIFE, short for Living Independence For the Elderly, opened April 2 at 303 13th Ave. East.
The program follows a nationally recognized model dubbed PACE, or Program of All-Inclusive Care for the Elderly, that combines primary care, therapies, skilled care, social and emotional support, and day care at the Empath LIFE facility along with in-home care and support.
